falkor « Thu May 20, 2010 3:43 pm » 1 magners and 1 cocoa cola, shaken not stirred plus Pork scratchings for you ukM and as requested dried prunes for you Skipper |
ukmedic1 « Thu May 20, 2010 3:43 pm » we get quite a few that are nothing short of personal attacks...
Skipper « Thu May 20, 2010 3:43 pm » ah thanks ukm, same here phecc (hpc) audit patient care reports and will direct medic to take more training or maybe do a hospital placement where they can practice the skill alot
ukmedic1 « Thu May 20, 2010 3:42 pm » complaints
ukmedic1 « Thu May 20, 2010 3:42 pm » yes - we are audited on all extended skills, and are expected to develop a remedial plan if we dont get a chance to use certain skills
|
falkor « Thu May 20, 2010 3:42 pm » so what is ambulance crew's biggest fear out there? assault? exposure to infection? what are they truly worried about? |
ukmedic1 « Thu May 20, 2010 3:41 pm » probably beyond in some fields..
Skipper « Thu May 20, 2010 3:41 pm » ukm are you monitiored for what intervention you preform and dont preform and if you dont use it a certain amount of times you have to go back to do refresher training??
falkor « Thu May 20, 2010 3:40 pm » cor glad to hear you say that ukM, must be pretty near Drs by now
ukmedic1 « Thu May 20, 2010 3:40 pm » there is also the problem of exposure - how often will you use a new skill?
ukmedic1 « Thu May 20, 2010 3:40 pm » probably not a lot...
falkor « Thu May 20, 2010 3:39 pm » ambulance crew have seen a revolution in training and responsibility since the stoneage 1960s, surely they must now be at a saturation level? how much more training and responsibility can ambulance crew take?
|
Skipper « Thu May 20, 2010 3:39 pm » yep need to recheck the nuts after about 10-15miles, not ideal when youre blueing it to a call
ukmedic1 « Thu May 20, 2010 3:38 pm » A lot of tyre companies say not to push it for 30 miles or so after changing a wheel...
ukmedic1 « Thu May 20, 2010 3:38 pm » but if five minutes later you are pushing it to its limits....
Skipper « Thu May 20, 2010 3:38 pm » oh well if were going alcoholic a magners for me please
Skipper « Thu May 20, 2010 3:38 pm » our ambo has a spare but no jack or spanner, we just give the aa a bell or else theres a fast fit opposite the fire station where we park it
ukmedic1 « Thu May 20, 2010 3:37 pm » changing the tyre is not so bad...
falkor « Thu May 20, 2010 3:37 pm » true so true ok umm lager then !!
ukmedic1 « Thu May 20, 2010 3:37 pm » bit warm for cocoa...
falkor « Thu May 20, 2010 3:37 pm » god who can't change a sodding tyre now? I do not believe this
Skipper « Thu May 20, 2010 3:37 pm » if not tea, 3 sugars please
Skipper « Thu May 20, 2010 3:36 pm » any coca cola falk??
ukmedic1 « Thu May 20, 2010 3:36 pm » there is also the risk involved in changing the tyre correctly...
falkor « Thu May 20, 2010 3:36 pm » oh I am sorry I haven't even offered the Jaffa Cakes, in amongst the excitement, umm ukM cocoa for you I believe and Skipper lukewarm Tizer for you?
ukmedic1 « Thu May 20, 2010 3:36 pm » We don't carry spares, as the space can be better used...
ukmedic1 « Thu May 20, 2010 3:36 pm » Loads of debate about what to do with flat tyres...
ukmedic1 « Thu May 20, 2010 3:35 pm » where a life is at immediate risk, or where only minor treatment / support is needed
falkor « Thu May 20, 2010 3:35 pm » the ambulance crew called a tow truck and that's nuts
ukmedic1 « Thu May 20, 2010 3:35 pm » mostly in the two extreemes of the spectrum...
Skipper « Thu May 20, 2010 3:35 pm » im sure theyll be more prominent after the investigation i link will be completed
ukmedic1 « Thu May 20, 2010 3:35 pm » Cars / bikes / cylces all have a role to play...
ukmedic1 « Thu May 20, 2010 3:35 pm » distinct from CPIs (Clinical performance indicators ) which measure what we do and the difference it makes...
falkor « Thu May 20, 2010 3:34 pm » an ambo car had a tyre let down the other day
ukmedic1 « Thu May 20, 2010 3:34 pm » KPIs are generally about times and availability...
Skipper « Thu May 20, 2010 3:34 pm » ah right, im not sure how thats recorded here, im not in dfb or hse, i work for private medic company and a vol so were not bound by performance indicators are is mostly event cover we do
ukmedic1 « Thu May 20, 2010 3:34 pm » and then get the political clout to get something done about it
ukmedic1 « Thu May 20, 2010 3:33 pm » If they are all stuck with patients at the hospital, then you can clearly show that your response times are being affected by others
falkor « Thu May 20, 2010 3:33 pm » what are KPIs on single manned ambo BIKES? I was going to ask you about the single manned cars but heeeey
ukmedic1 « Thu May 20, 2010 3:33 pm » If the time to get a vehicle moving is long, then look where the vehicles are.
falkor « Thu May 20, 2010 3:33 pm » oh sure
ukmedic1 « Thu May 20, 2010 3:32 pm » Such as time to answer a call, time to get a vehicle moving, time to scene...
Skipper « Thu May 20, 2010 3:32 pm » what be them??
falkor « Thu May 20, 2010 3:32 pm » can you give an example of one ukM?
ukmedic1 « Thu May 20, 2010 3:32 pm » Key Performance Indicators...
Skipper « Thu May 20, 2010 3:31 pm » KPI's???
ukmedic1 « Thu May 20, 2010 3:31 pm » If you show that the reason is outside your control - such as hospital delays - then start complaining
falkor « Thu May 20, 2010 3:31 pm » yikes it looks like a TANK, yeah with you now Skip
ukmedic1 « Thu May 20, 2010 3:31 pm » If you don't hit a target, examine why...
ukmedic1 « Thu May 20, 2010 3:30 pm » Hey skipper - don't be afraid of KPIs - make them work for you....
Skipper « Thu May 20, 2010 3:30 pm » the rig in ukmedics link
falkor « Thu May 20, 2010 3:30 pm » than what? I lost the thead of this
Skipper « Thu May 20, 2010 3:30 pm » jaysus, ive seen houses smaller than that
falkor « Thu May 20, 2010 3:29 pm » chevvys?
ukmedic1 « Thu May 20, 2010 3:29 pm » I spend a week working in Austin - Just fell in love with thier vehicles!
falkor « Thu May 20, 2010 3:29 pm » disgusting but then again immigration is rampant so what she we expect
ukmedic1 « Thu May 20, 2010 3:28 pm » have a look at : http://atcemsce.org/home/index.php?opti ... &Itemid=28
Skipper « Thu May 20, 2010 3:28 pm » if eighter of you are ever bored give this a read http://www.medicalindependent.ie/page.a ... _emergency
ukmedic1 « Thu May 20, 2010 3:28 pm » just mst are built for big american roads, so would not cope with London streets...
Skipper « Thu May 20, 2010 3:28 pm » according to the nursing union in my local hospital and the one where i do placement last night there were 33patients on trolleys in a&e
ukmedic1 « Thu May 20, 2010 3:27 pm » If you want a really nice vehicle there are much better than the Chevy...
falkor « Thu May 20, 2010 3:27 pm » what is your take on single manned accident "cars"?
|
falkor « Thu May 20, 2010 3:27 pm » that whole thing makes me wince
Skipper « Thu May 20, 2010 3:26 pm » ah see in my vol we dont ask for bandages back just ask ambo crew if they have any spare to give us since we buy our own kit or else pilfer from a&e if we transport
ukmedic1 « Thu May 20, 2010 3:26 pm » We now report them as serious untoward incidents if the crew wait over an hour - it upsets the hospitals, but seems to be reducing the delays
falkor « Thu May 20, 2010 3:26 pm » too many people flooding them
ukmedic1 « Thu May 20, 2010 3:26 pm » Hospital delays are a nightmare...
|
falkor « Thu May 20, 2010 3:25 pm » chevvys!! |
ukmedic1 « Thu May 20, 2010 3:25 pm » We need someone to build ambulances from scratch again...
falkor « Thu May 20, 2010 3:25 pm » oh god that reminds me of the St Johns lot wanting their bandages back, bad memories
ukmedic1 « Thu May 20, 2010 3:25 pm » then came the Mercs- again quite popular, but BIG differences in the conversion from different bodybuilders..
Skipper « Thu May 20, 2010 3:24 pm » and with only around 16-17 999 ambos on the road most days for 1.5-2.5million people, alot of rigs are tied up waiting on their stretcher back!
ukmedic1 « Thu May 20, 2010 3:24 pm » Back to the ambulances, we then had UVM modular bodies on the LDV - a nice vehicle to work with, but LDV kept changing the base vehicle, so no two years were ever the same
falkor « Thu May 20, 2010 3:24 pm » 6 speed auto ? fabbo fabbo - teas made onboard as well? air con?
Skipper « Thu May 20, 2010 3:23 pm » Theres just no room in a&e for patients falk
falkor « Thu May 20, 2010 3:23 pm » ambo crewsstuck in a 2 hour queeue in A+E? how can that be sodding well allowed? insane
ukmedic1 « Thu May 20, 2010 3:23 pm » so five is a nice number...
ukmedic1 « Thu May 20, 2010 3:23 pm » and one pmanaging the overal scene
Skipper « Thu May 20, 2010 3:23 pm » for a long time everyone used transits with wilker bodywork here, recently hse changed to merc sprinters.....about 8 or 9 burst into flames, since sorted i think, dfb have just changed to merc sprinters 6 speed auto with browns bodywork and love them
ukmedic1 « Thu May 20, 2010 3:23 pm » one playing with the drugs,
ukmedic1 « Thu May 20, 2010 3:23 pm » One persoon managing the airway
ukmedic1 « Thu May 20, 2010 3:23 pm » Extra hands are great. Good evidence to show quality of CPR drops quickly after 2 mins, so you need to be able to swap.
ukmedic1 « Thu May 20, 2010 3:22 pm » (were the J reg still Leyland?)
Skipper « Thu May 20, 2010 3:22 pm » just on your bit on arrests ukmedic, sorry to keep going back to this but when dfb recieve a call for an arrest, they send a fire appliance too as a) is often on scene quicker b) crewed by dual qualified ff's/paramedics c) aed and bls bag on board d) extra hands to help with resus on the ambo
ukmedic1 « Thu May 20, 2010 3:21 pm » Then came the LDV vans - absolute nightmare - the manuals were difficult to drive, and the autos jumped between gears....
|
falkor « Thu May 20, 2010 3:21 pm » yeah but they looked amazing COME ON |
ukmedic1 « Thu May 20, 2010 3:21 pm » The big chevys - not a good buy!
ukmedic1 « Thu May 20, 2010 3:20 pm » Hanlon and a couple of other companies built ambulances on the Bedford chasis, but they were never the same....
falkor « Thu May 20, 2010 3:20 pm » did you ever see the big old American job issued to Surrey?
ukmedic1 « Thu May 20, 2010 3:20 pm » Well I started driving the Bedford CF - that was a good vehicle, but they stopped making them...
Skipper « Thu May 20, 2010 3:20 pm » take the hospital i do my placements for uni in, theyve just closed a 5 day wars (32 beds) another 30 accross the hospital to save on wages, but, and its a big BUT, its the one of the busiest a&e's in the country, often 2+ hour wait for ambo crews due to a lack of trolleys, trolleys occupied by people waiting to be admitted to a bed, less beds = bigger queues = less ambos on the road
falkor « Thu May 20, 2010 3:19 pm » let's talk ambulances, can you chat over the different makes of 999 ambulances that you have driven?
ukmedic1 « Thu May 20, 2010 3:19 pm » It might be two or threee minutes before someone realises that calling an ambo is a good idea!
falkor « Thu May 20, 2010 3:18 pm » Skipper any examples from you pally?
ukmedic1 « Thu May 20, 2010 3:18 pm » for ambos to have any impact on cardiac arrest we need to be there within six mins - it simply does not happen
falkor « Thu May 20, 2010 3:18 pm » damn good example
falkor « Thu May 20, 2010 3:18 pm » training cannot be mothballed - 1 year later it would have to be reviewed to see what needs updating
ukmedic1 « Thu May 20, 2010 3:18 pm » Elderly lady who collapsed, and was given CPR by a bystander (first aider on a building site) kept her going till we got there
Skipper « Thu May 20, 2010 3:17 pm » there is no foresight in the irish public service
Skipper « Thu May 20, 2010 3:17 pm » yes well its desk jockeys justifying their existence by saving money.......only in the short term though, training is still done at station level but stuff like recruit training specialist courses etc have stopped
ukmedic1 « Thu May 20, 2010 3:17 pm » Early actions - yes...
ukmedic1 « Thu May 20, 2010 3:16 pm » esp if you then have to pay OT to keep vehicles on the road!
ukmedic1 « Thu May 20, 2010 3:16 pm » Skipper - we have a constant battle to preserve training - mothballing it does not make economic sense
|
falkor « Thu May 20, 2010 3:15 pm » jees - didn't know about this pension levy, completely missed it, oh dear anyway ukm in one post you said "It is the early actions of those nearby that make the difference if you live or die" have you had an example of that lately? |
ukmedic1 « Thu May 20, 2010 3:15 pm » mixture of problems I think - not least publication of would should have remained sensitive info in quite a lot of places
falkor « Thu May 20, 2010 3:14 pm » sounds good to me, wasn't it moderated properly? what was the problem?
Skipper « Thu May 20, 2010 3:14 pm » well last year before the emegency budget the govt decided to apply a levy to public service pensions, and as a result divided the country between public and private sector, the only thing they ever did right was to turn the two against each other
ukmedic1 « Thu May 20, 2010 3:14 pm » no proper clinical discussions ever took place
ukmedic1 « Thu May 20, 2010 3:14 pm » big white taxi service was essentially a place for people to vent frustrations..
falkor « Thu May 20, 2010 3:13 pm » dreadful
Skipper « Thu May 20, 2010 3:13 pm » dfb have had to mothball training due to budget cuts despite having a panel of about 150 recruits awaiting training, so alot of lads need to do ot to keep appliances and ambos on the road
|
falkor « Thu May 20, 2010 3:13 pm » so the white taxi service? i heard about that one - never saw it in action though |
ukmedic1 « Thu May 20, 2010 3:13 pm » Interstingly at least two of them have gone off line recently
falkor « Thu May 20, 2010 3:12 pm » pension levy? what the hell is that?
ukmedic1 « Thu May 20, 2010 3:12 pm » Why did I come here? I simply did not like some of the other sites...
Skipper « Thu May 20, 2010 3:12 pm » no, lot of retirements due to recent pension levy, dfb is down a couple of hundred men, im sure the hse is worse, infact theyve no chief ambo officer at the moment!
falkor « Thu May 20, 2010 3:11 pm » do you think it's sufficiently manned Skipper?
Skipper « Thu May 20, 2010 3:11 pm » luckily ambulance service was sparred the civil service recruitment moratorium by the government here
|
falkor « Thu May 20, 2010 3:11 pm » ukM this site kicked off just over a year ago, why did you agree to be part of the team to keep it going? what attracted you to this venture? |
|
falkor « Thu May 20, 2010 3:10 pm » bad days ahead |
ukmedic1 « Thu May 20, 2010 3:10 pm » all of our vacancies now go on NHS jobs...
ukmedic1 « Thu May 20, 2010 3:10 pm » limited options at the moment here...
Skipper « Thu May 20, 2010 3:10 pm » Yup
falkor « Thu May 20, 2010 3:09 pm » in Dublin?
Skipper « Thu May 20, 2010 3:09 pm » Order of Malta Ambulance Corps
falkor « Thu May 20, 2010 3:09 pm » do you happen to know what the recruitment situation is for LAS at this time ?
|
Skipper « Thu May 20, 2010 3:09 pm » as for the hse i have no idea about their rank structure
Skipper « Thu May 20, 2010 3:08 pm » DFB in the main only firefighters are clincal medics, some ap's are sub officers anything above that is firebrigade ranks in management role
ukmedic1 « Thu May 20, 2010 3:08 pm » which gives a chain of command that is obvious, but might not reflect what they actually do?
falkor « Thu May 20, 2010 3:08 pm » what IS your voluntary org?
Skipper « Thu May 20, 2010 3:08 pm » well in my voluntary org we follow military ranks loosely start at volunteer, then corporal, sgt, adjutant, lieutenant, captain, commander etc etc
ukmedic1 « Thu May 20, 2010 3:07 pm » At least Clinical Team Leader is what the job says it is...
ukmedic1 « Thu May 20, 2010 3:06 pm » Then you have a duty officer / manager - theit role is to manage, not always clinical.....
falkor « Thu May 20, 2010 3:06 pm » cor rubbish rank titles for definite, Skipper or your lot any different?
ukmedic1 « Thu May 20, 2010 3:06 pm » leading paramedic / team leader... all variations on a theme
ukmedic1 « Thu May 20, 2010 3:05 pm » Most will have a clinical team leader, who overseas a group of medics, ensuring that they are doig the job correctly...
falkor « Thu May 20, 2010 3:05 pm » is it leading paramedic next?
ukmedic1 « Thu May 20, 2010 3:05 pm » It varies a little bit between services...
falkor « Thu May 20, 2010 3:04 pm » how do paramedics get promoted ukm? what is the next rank up and how do they get it?
|
Skipper « Thu May 20, 2010 3:04 pm » I know ukmedic, my father is a customs officer in drugs enforcement unit of dublin airport, even they estimate they only get 10% of what comes in, its an internationally reconised precentage
ukmedic1 « Thu May 20, 2010 3:04 pm » Wants they have a stranglehold, breaking the grip is almost impossible....
falkor « Thu May 20, 2010 3:03 pm » well yeah I can't say I disagree with that,look at Mexico, complete chaos
ukmedic1 « Thu May 20, 2010 3:03 pm » There have been a few experiments rounding up the users - results are mixed
ukmedic1 « Thu May 20, 2010 3:03 pm » Most enforcement now is aimed at the trafficers and pushers...
falkor « Thu May 20, 2010 3:03 pm » oh I see, hmmm yeah ok but this is easy to say perhaps
ukmedic1 « Thu May 20, 2010 3:02 pm » step back from the patient - It is rare that you need to be in the path of a swinging arm of an otherwise unconscious patiet
Skipper « Thu May 20, 2010 3:02 pm » Falk i was thinking that the other night, ive started a now job with an event medic company, mostly night club work, drugs are bound to be a mitigating factor in a large portion of cases, i dont think there is an aswer to thr drug problem, legalisation will end in disaster and enforcement will only take about 10% off the streets
ukmedic1 « Thu May 20, 2010 3:02 pm » I see a crazy amount of drug use - personally I think that we are going to have to start clamping down on users a lot more...
falkor « Thu May 20, 2010 3:01 pm » step back from what?
ukmedic1 « Thu May 20, 2010 3:01 pm » Is this really an assault, or is it the medic not learning to step back?
falkor « Thu May 20, 2010 3:00 pm » what's your thought on the world wide endemic use of cocaine, heroin, cannabis - reading the papers and watching the TV it is obvious that a large proportion of the population in the UK and across the world love to stuff down copious amounts of controlled drugs (sometimes whether they can afford them or not) what IS the answer?
ukmedic1 « Thu May 20, 2010 3:00 pm » The diabetic or epileptic who is disorientated as they come round may lash out...
Skipper « Thu May 20, 2010 3:00 pm » and i found out today im doing a 3 week placement for college in a&e in sept, cant wait
ukmedic1 « Thu May 20, 2010 3:00 pm » True physical violence is mostly drink or drug related, with some being the effect of a medical complaint
Skipper « Thu May 20, 2010 3:00 pm » hostile places at the best of times
falkor « Thu May 20, 2010 3:00 pm » why?
Skipper « Thu May 20, 2010 3:00 pm » a&e depts all over are like that
ukmedic1 « Thu May 20, 2010 3:00 pm » A lot of verbal aggression in almost every dept in London every day...
ukmedic1 « Thu May 20, 2010 2:59 pm » not sure why the injury pattern is diffeerent, but it seems it is
|
falkor « Thu May 20, 2010 2:59 pm » ukM what is that state of play with London A + Es nowadays? |
ukmedic1 « Thu May 20, 2010 2:59 pm » Chest and back wounds in coppers are well documented...
ukmedic1 « Thu May 20, 2010 2:58 pm » Most knife attacks on uk ambo staff have been to areas not covered by the vests
falkor « Thu May 20, 2010 2:58 pm » same with Police TBH but then - better to be safe than sorry
Skipper « Thu May 20, 2010 2:58 pm » ive only ever seen medics with stabbies in the uk, nowhere else
ukmedic1 « Thu May 20, 2010 2:58 pm » very little evidence to show that they have saved any injuries at all...
falkor « Thu May 20, 2010 2:58 pm » skipper are your crews in stab vests?
ukmedic1 « Thu May 20, 2010 2:58 pm » A bit of over-reaction...
falkor « Thu May 20, 2010 2:58 pm » how crazy is that?
ukmedic1 « Thu May 20, 2010 2:57 pm » seems we led the world on that one
ukmedic1 « Thu May 20, 2010 2:57 pm » No - they were very suprised that we had stab vests..
